1. Confidentiality & Privacy

To protect your privacy:

  • Your personal information is protected under BC’s Personal Information Protection
  • Act (PIPA) and the BCACC Code of Ethical Conduct.
  • Limits to Confidentiality: Information may be shared only if:
    • There is risk of harm to yourself or others.
    • Required by law (e.g., court orders, child protection).
    • You file a complaint with the BCACC (your records may be reviewed).

2. Informed Consent

You must consent to:

  • Collaboration on goals, methods, and progress.  You may withdraw consent anytime.
  • Digital Records: Secure platforms (e.g. Jane) store your data with encryption.

3. Dual Roles & Boundaries

To avoid conflicts of interest:

  • While we recognize the close-knit nature of small communities like Revelstoke, we uphold clear professional boundaries to ensure your care remains unbiased and effective. This means we avoid dual relationships (e.g., social, financial, or other roles outside therapy) that could conflict with your therapeutic needs.
  • If prior or unavoidable relationships exist (e.g., small communities), we will discuss this openly and document safeguards.
  • Exceptions: Court-ordered assessments require explicit role clarity; counselling and assessment services will not overlap without your informed consent.

4. Record Keeping

Records are retained for:

  • 10+ Years (longer for minors)
  • You may request access to your file in writing

5. Fees & Cancellations

At our practice:

  • Sessions are paid at the time of service.
  • 24+ hours' notice is required for cancellations to avoid charges.

6. Complaint Process

Concerns may be directed to:

  • Kaizen Wellness Centre Ltd (us) OR
  • BCACC

NOTE: Complaints involve sharing your records with the BCACC.

7. Discontinuation of Services

Your care is discontinued if:

  • You request to discontinue care at any time.
  • After 3+ months of inactivity, your file may be closed with documented outreach.

8. Inclusive Care

We honour your cultural, gender, and identity needs:

  • Discrimination is NOT tolerated and you may either discontinue services and/or file a complaint with Kaizen Wellness Centre Lt or the BCACC if at any point you feel discriminated against by any of our counsellors.
